Application packets include:

__	Application Form (3 pages)
__	Study Abroad Condidtions of Participation (2 pages)
__	UI Medical Self-Assessment (2 pages)
__	Statement of Medical Insurance Coverage (1 page)
__	A copy of the photo page of your passport is also needed for the group visa. The copy must be clear and legible.
